Кардиоваскулярный риск у больных воспалительными артритами

При ревматоидном и других воспалительных артритах (анкилозирующий спондилит и псориатический артрит) наблюдается высокий риск сердечно-сосудистых (кардиоваскулярных) заболеваний (ССЗ). Он обусловлен ускоренным развитием атеросклероза, ассоциированным с хроническим системным воспалительным процессом. Тем не менее традиционные факторы риска ССЗ (артериальная гипертензия, курение, дислипидемия) также имеют значение для больных воспалительными артритами. Наибольшее количество данных накоплено в отношении взаимосвязи ССЗ и ревматоидного артрита. В связи с трудностями диагностики ишемической болезни сердца и других ССЗ большое значение приобретает выявление пациентов высокого и очень высокого риска. Применение шкал оценки суммарного сердечно-сосудистого риска SCORE/SCORE2 с коэффициентом 1,5 позволяет выявить пациентов, нуждающихся в проведении мероприятий по снижению высокого риска ССЗ. Контроль воспалительной активности заболевания, изменение образа жизни, применение статинов и антигипертензивных препаратов в соответствии с рекомендациями, осторожность при назначении нестероидных противовоспалительных препаратов и минимизация дозы глюкокортикоидов – основные компоненты стратегии снижения риска ССЗ у больных воспалительными артритами.
