All patients were divided into 3 groups according to left ventricular ejection fraction, followed by an analysis of patient characteristics.</p> <p><bold>Results.</bold> A total of 486 patients were included (55 % men); mean age was 72 ± 12 years. The number of patients with heart failure with reduced ejection fraction was 235 (48 %), with mildly reduced ejection fraction – 117 (24 %), and with preserved ejection fraction – 134 (28 %). More than 60 % of patients required admission to the intensive care unit. The groups differed in sex, age, presence of coronary artery disease, postinfarction cardiosclerosis, history of myocardial revascularization, presence of infection at admission, as well as in physical examination findings, laboratory and instrumental parameters; however, no differences in mortality were observed between the groups (<italic>p</italic> &gt;0.05).</p> <p><bold>Conclusion.</bold> Despite the fact that patients with heart failure with reduced ejection fraction had more severe condition at admission per clinical, laboratory, and instrumental parameters, more often had a history of coronary artery disease, postinfarction cardiosclerosis, and myocardial revascularization, more frequently required inotropic / vasopressor support, and had longer hospital stays, mortality rates were comparable between the groups.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> – оценить клинико-демографические, лабораторно-инструментальные характеристики, терапию и исходы у пациентов с острой декомпенсацией сердечной недостаточности в зависимости от фракции выброса (ФВ) левого желудочка.</p> <p><bold>Материал и методы.</bold> Проведено сплошное ретроспективное исследование с включением пациентов &gt;18 лет, госпитализированных с острой декомпенсацией сердечной недостаточности в многопрофильный стационар в период с 01.12.2019 по 01.12.2021 Пациентов разделили на 3 группы в зависимости от ФВ левого желудочка, после чего провели анализ характеристик пациентов.</p> <p><bold>Результаты.</bold> В исследование включены 486 пациентов (55 % мужчин), средний возраст которых составил 72 ± 12 лет. Число пациентов с хронической сердечной недостаточностью с низкой ФВ достигло 235 (48 %), с умеренно сниженной ФВ – 117 (24 %), с сохраненной ФВ – 134 (28 %). Более 60 % пациентов нуждались в госпитализации в отделение реанимации и интенсивной терапии. Группы пациентов различались по полу, возрасту, частоте сопутствующей ишемической болезни сердца, наличию постинфарктного кардиосклероза, реваскуляризации миокарда в анамнезе, наличию инфекционного заболевания при поступлении, показателям физикального осмотра, лабораторным и инструментальным данным, но не по летальности (<italic>p</italic> &gt;0,05).</p> <p><bold>Заключение.</bold> Несмотря на то что пациенты с хронической сердечной недостаточностью с низкой ФВ при поступлении имели более тяжелое клиническое состояние, неудовлетворительные лабораторные показатели и результаты инструментальных исследований, чаще имели в анамнезе ишемическую болезнь сердца, постинфарктный кардиосклероз и реваскуляризацию миокарда, чаще нуждались в инотропной / вазопрессорной поддержке и дольше находились в стационаре, показатели летальности в группах оказались сопоставимы.</p></trans-abstract><kwd-group xml:lang="en"><kwd>acute decompensated heart failure</kwd><kwd>chronic heart failure</kwd><kwd>myocardial injury</kwd><kwd>troponin</kwd><kwd>mortality</kwd><kwd>biomarker</kwd><kwd>left ventricular ejection fraction</kwd><kwd>echocardiography</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>острая декомпенсация сердечной недостаточности</kwd><kwd>хроническая сердечная недостаточность</kwd><kwd>повреждение миокарда</kwd><kwd>тропонин</kwd><kwd>летальность</kwd><kwd>биомаркер</kwd><kwd>фракция выброса левого желудочка</kwd><kwd>эхокардиография</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Shlyakhto E.V., Belenkov Yu.N., Boytsov S.A. et al.
