Comprehensive laboratory and instrumental diagnostics were performed, and therapy with nonsteroidal anti-inflammatory drugs in combination with colchicine was initiated, followed by an assessment of the effectiveness of the administered therapy.</p> <p><bold>Results.</bold> Against the background of combined anti-inflammatory therapy with ibuprofen 800 mg three times daily and colchicine 0.5 mg twice daily for 4 months, a sustained clinical and laboratory effect was achieved, including clinical improvement, normalization of C-reactive protein levels, and regression of pericarditis manifestations according to echocardiographic data.</p> <p><bold>Conclusion.</bold> This case highlights that post-interventional pericarditis may occur even after elective percutaneous coronary intervention without overt mechanical complications and may become recurrent. Early recognition of post-cardiac injury syndrome, differential diagnosis with other conditions, and the use of prolonged colchicine-containing anti-inflammatory therapy are required.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель работы</bold> – продемонстрировать особенности клинического течения и терапии рецидивирующего перикардита как варианта синдрома поврежденного сердца после чрескожного коронарного вмешательства.</p> <p><bold>Материал и методы.</bold> Пациент Щ., 76 лет, с ишемической болезнью сердца. После планового чрескожного коронарного вмешательства с попыткой реканализации хронической окклюзии правой коронарной артерии развился экссудативно-фибринозный перикардит с последующим рецидивом. Проведена комплексная лабораторно-инструментальная диагностика, инициирована терапия нестероидными противовоспалительными препаратами с колхицином с последующей оценкой эффективности проводимой терапии.</p> <p><bold>Результаты.</bold> На фоне проводимой комбинированной противовоспалительной терапии ибупрофеном по 800 мг 3 раза в сутки и колхицином по 0,5 мг 2 раза в сутки на протяжении 4 мес достигнут стойкий клинико-лабораторный эффект в виде клинического улучшения, нормализации уровня С-реактивного белка и регресса проявлений перикардита по данным эхокардиографии.</p> <p><bold>Заключение.</bold> Постинтервенционный перикардит может возникать после планового чрескожного коронарного вмешательства без явных механических осложнений и приобретать рецидивирующее течение.
