The study methodology included assessment of the radiographic stage of OA, body mass index, age, duration of the disease, degree of pain catastrophizing using the Pain Catastrophizing Scale (PCS). Patients answered questions of short form McGill Pain Questionnaire – 2 (SF-MPQ2), Hospital Anxiety and Depression Scale, and Visual Analogue Scale for evaluating pain symptoms.</p> <p><bold>Results.</bold> Three groups were identified based on levels of pain catastrophizing: 1<sup>st</sup> group (33.7 %) had minimal deviations (&lt; 14 points), 2<sup>nd</sup> group (42.9 %) showed moderate deviations (14–26 points); 3<sup>rd</sup> group (23.5 %) exhibited significant deviations (&gt;26 points). Patients from the third group rated their pain higher (2.7 out of 5 points) and used more qualitative descriptors according to the SF-MPQ2. This group also reported more frequent use of nonsteroidal anti-inflammatory drugs (73.9 %). Higher anxiety and depression scores were observed in patients with high catastrophizing rates. Pain in this group was characterized by neuropathic features. Regression analysis revealed that anxiety was the strongest predictor of catastrophizing. Other significant factors included body mass index, number of selected pain characteristics, and overall pain intensity measured by SF-MPQ2.</p> <p><bold>Conclusion.</bold> Perception of pain in OA depends not only on structural changes but also on psychosocial factors, particularly pain catastrophizing. Comprehensive assessment incorporating psychological and behavioral traits enables personalization of therapy and potentially improves the outcomes.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> – оценить частоту встречаемости и выраженность катастрофизации боли среди пациентов с остеоартритом (ОА) коленного сустава и установить влияние катастрофизации на характеристики боли.</p> <p><bold>Материал и методы.</bold> В исследование включены 98 пациентов с ОА коленного сустава (93,8 % – женщины). Методология исследования включала оценку рентгенологической стадии ОА, индекса массы тела, возраста, длительности заболевания, уровня катастрофизации боли по PCS (Pain Catastrophizing Scale, шкала катастрофизации боли). Пациентам предлагалось ответить на вопросы SF-MPQ2 (Short Form of the McGill Pain Questionnaire – 2, краткая форма опросника боли Макгилла – 2), Госпитальной шкалы тревоги и депрессии и визуальной аналоговой шкалы для оценки боли.</p> <p><bold>Результаты.</bold> По уровню катастрофизации боли по PCS выделено 3 группы пациентов: 1-я (33,7 %) – больные с минимальными отклонениями (&lt; 14 баллов), 2-я (42,9 %) – с умеренными (14–26 баллов), 3-я (23,5 %) – с выраженными (&gt;26 баллов) отклонениями. По SF-MPQ2 пациенты 3-й группы оценивали боль выше (2,7 из 5 баллов) и использовали больше качественных характеристик боли. В этой же группе чаще (73,9 %) применялись нестероидные противовоспалительные препараты. У пациентов с высокой катастрофизацией выявлены более высокие уровни тревоги и депрессии. Характер боли в этой группе отличался наличием нейропатических характеристик. По результатам регрессионного анализа сильнейшим предиктором катастрофизации оказалась тревога. Значимыми факторами также были индекс массы тела, количество выбранных характеристик и выраженность боли по SF-MPQ2.</p> <p><bold>Заключение.</bold> Восприятие боли при ОА определяется не только структурными изменениями суставов (рентгенологической стадией болезни), но и психоэмоциональными факторами, среди которых особое значение имеет катастрофизация боли.
