Patients with a follow-up duration of less than 6 months, those who had not undergone conventional disease-modifying antirheumatic drug (DMARD) therapy, and patients later reclassified with a different diagnosis were excluded. In the selected group (<italic>n </italic>= 966) DtT patients were identified as those with RA who received recommended treatment with conventional DMARD but failed to achieve LDA or remission after therapy with two classes of biologic disease-modifying antirheumatic drugs (bDMARDs) or janus kinase inhibitors (JAKi)/bDMARDs, or had their treatment discontinued due to adverse events. In the DtT group, two subgroups were distinguished: those who achieved LDA after being classified as DtT due to a change in JAKi/bDMARDs and those who did not achieve LDA.</p><p><bold>Results. </bold>Median duration of the disease in the DtT cohort was 186 months (interquartile range 104–278), and median duration of DtT status at the time of the last observation was 27.8 months (interquartile range 10.2–56.0 months). A total of 484 treatment episodes involving JAKi/bDMARDs among DtP patients were analyzed. Comparison of the group that achieved LDA (LDA+, <italic>n </italic>= 172; 35.5 %) with the group that did not achieve LDA (LDA–, <italic>n </italic>= 312; 64.5 %) revealed that the LDA+ was characterized by younger age of RA onset (<italic>p </italic>= 0.005), later initiation of DMARD (<italic>p </italic>= 0.005) and JAKi/bDMARDs (<italic>p </italic>= 0.034) therapies from the onset of RA, and more frequent presentation with stage IV radiological findings per Steinbroker (<italic>p </italic>= 0.007). Additionally, glucocorticoids were used less frequently at the last visit in the LDA+ group (<italic>p </italic>= 0.042). Comparison of the frequency of LDA or remission within the DtP cohort revealed no significant differences among classes or individual medications. In the highest percentage of cases, abatacept and IL-6 inhibitors achieved target disease activity (40.5 % and 40.4 % respectively). Among the IL-6 inhibitors, the highest success rates were observed with levilimab (48.0 %) and tocilizumab (41.3 %).</p><p><bold>Conclusion. </bold>Patients with late-stage RA and pronounced radiological changes show potential for achieving LDA. The administration of abatacept and IL-6 inhibitors as first-line therapies may reduce the likelihood of developing DtT status, and in cases with this status occurs, target disease activity can be achieved in 40.5 % and 40.4 % of cases, respectively.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования </bold>– изучить предикторы достижения низкой активности заболевания (НАЗ) у пациентов с труднолечимым ревматоидным артритом (РА) и оценить роль ингибиторов интерлейкина 6 (иИЛ-6) в достижении целевой активности заболевания.</p><p><bold>Материал</bold><bold> </bold><bold>и методы.</bold><bold> </bold>проведен анализ медицинской документации 1145 пациентов с достоверным диагнозом РА. Исключены пациенты с длительностью наблюдения менее 6 мес, не принимавшие базисную терапию, а также пациенты с последующей сменой диагноза. В отобранной группе (<italic>n </italic>= 966) выделены труднолечимые пациенты, к которым относили больных РА, получавших рекомендуемое лечение традиционными базисными противовоспалительными препаратами, и у которых терапия 2 классами генно-инженерных биологических препаратов (ГИбп) или ингибиторов янус-киназ (иЯК) не позволила достичь НАЗ или ремиссии или была прекращена из-за нежелательных явлений.</p><p>В группе труднолечимых больных выделены 2 подгруппы: пациенты, которые после установления труднолечимости достигли НАЗ в результате смены ГИбп/иЯК и которые не достигли низкой степени активности РА.</p><p><bold>Результаты. </bold>Медиана длительности болезни в когорте труднолечимых пациентов составила 186 мес (межквартильный размах 104–278 мес), медиана длительности труднолечимости на момент последнего наблюдения – 27,8 мес (межквартильный размах 10,2–56,0 мес). проанализированы 484 эпизода терапии труднолечимых пациентов при помощи ГИбп/иЯК. при сравнительном анализе групп, достигших НАЗ (НАЗ+, <italic>n </italic>= 172; 35,5 %) и не достигших НАЗ (НАЗ–, <italic>n</italic><italic> </italic>= 312; 64,5 %), выявлено, что в группе НАЗ+ наблюдались более молодой возраст дебюта РА (<italic>р </italic>= 0,005), более позднее начало лечения базисными противоревматическими препаратами (<italic>р </italic>= 0,005) и ГИбп/иЯК (<italic>р </italic>= 0,034) с момента дебюта РА, чаще выявлялась IV рентгенологическая стадия по Штейнброкеру (<italic>р </italic>= 0,007), реже использовались глюкокортикоиды на момент последнего визита (<italic>р </italic>= 0,042). при сравнении частоты достижения НАЗ или ремиссии когорты труднолечимых пациентов значимых различий между классами и отдельными препаратами не выявлено. В наибольшем проценте случаев применение абатацепта и иИЛ-6 позволяло добиваться целевой активности (40,5 и 40,4 % пациентов соответственно). Среди иИЛ-6 наибольший процент успеха демонстрировали левилимаб (48,0 %) и тоцилизумаб (41,3 %).</p><p><bold>Заключение. </bold>пациенты с поздней стадией РА и выраженными рентгенологическими изменениями перспективны в отношении достижения НАЗ РА.
