Quitting smoking benefits even after a cancer diagnosis regardless of location and stage of the disease. Treatment plans should include smoking cessation recommendations combining motivational strategies and behavioral therapy, pharmacotherapy based on evidence-based medicine with follow-up and re-treatment as needed. The physician can inform patients not motivated to quit smoking at the moment about the strategy of risk modification by switching to alternative nicotine delivery sources (ANDS as the general name for category of smokeless products) that exclude burning tobacco. In the future, patients who quit smoking traditional cigarettes, but use only ANDS, should be maintained constant smoking cessation and encouraged to abandon alternative sources of nicotine delivery.</p><p>Given the proven effectiveness of introducing smoking cessation recommendations into practice, it is proposed to develop, on the basis of scientifically based methods and international experience, an algorithm for consulting on smoking cessation assistance and include it in clinical recommendations for the head and neck cancer in section “Prevention and dispensary monitoring, medical indications and contraindications to the use of prevention methods”.</p></abstract><trans-abstract xml:lang="ru"><p>Представленные материалы экспертного совета (Москва, 29 марта 2024 г.) посвящены современным подходам к модификации риска табакокурения у пациентов с онкологическими заболеваниями органов головы и шеи, а также выбору оптимальной научно-обоснованной стратегии помощи в отказе от курения или способа минимизации воздействия табачного дыма для пациентов, не мотивированных на отказ от курения.</p><p>Прекращение курения является основным направлением профилактики онкологических заболеваний. Отказ от курения приносит пользу даже после постановки диагноза рака независимо от локализации и стадии заболевания. Планы лечения должны включать рекомендации по отказу от курения, сочетающие мотивационные стратегии и поведенческую терапию, фармакотерапию, основанные на данных доказательной медицины с последующим наблюдением и повторным лечением по мере необходимости. Врач может проинформировать пациентов, не мотивированных на отказ от курения в настоящий момент, о стратегии модификации рисков с помощью перехода на альтернативные источники доставки никотина (АИДН, общее название категории бездымных продуктов), исключающие горение табака. В дальнейшем у пациентов, отказавшихся от курения традиционных сигарет, но использующих исключительно АИДН, следует поддерживать постоянный отказ от курения и поощрять отказ и от АИДН.</p><p>С учетом доказанной эффективности внедрения в практику рекомендаций по отказу от курения предложено разработать на основе научно-обоснованных методов и международного опыта алгоритм проведения консультаций по помощи в отказе от курения и включить его в клинические рекомендации по раку органов головы и шеи в раздел «Профилактика и диспансерное наблюдение, медицинские показания и противопоказания к применению методов профилактики».</p></trans-abstract><kwd-group xml:lang="en"><kwd>head and neck cancer</kwd><kwd>smoking cessation</kwd><kwd>risk modification strategy</kwd><kwd>alternative source of nicotine delivery</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>онкологическое заболевание органов головы и шеи</kwd><kwd>отказ от курения</kwd><kwd>стратегия модификации рисков</kwd><kwd>альтернативный источник доставки никотина</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Gordiyenko V.P.
