For the immunological characteristic, all patients were tested for anti-topoisomerase (anti-Scl-70), and anti-centromeric (anti-CENP-B) antibodies, and anticentromere antibodies (anti-Pm-Scl).</p><p><bold>Results. </bold>The study included 79 patients with SSc. There was 94.9 % women. Average age – 64.4 ± 11.5 years. Signs of interstitial lung disease (ILD), according to HRCT were detected in 50 patients. The largest extent of lung injury was noted in patients with SSc sine scleroderma (32.7 ± 29.3 %), a smaller extent in patients with diffuse form SSc (16.9 ± 17.1 %) and the lowest in patients with limited SSc (8.5 ± 14.2 %). In addition to the type of disease, the extent of lung injury in patients with SSc-ILD was statistically significantly higher in patients with arthralgia, dyspnea and the presence of antibodies to topoisomerase I and combined autoantibodies. Also, a statistically significant feedback was established with all indicators of the test with a 6-minute walk and forced vital capacity and a direct relationship with indicators of pulmonary artery systolic pressure. When evaluating the correlation between the extent of lung injury and the degree of dyspnea according to Borg, it was found that in patients who assessed dyspnea less than 3 points, the extent of lung injury was less than 25 %. Due to the high degree of correlation, a regression formula was created for the dependence of the extent of lung injury on the distance in the test with a 6-minute walk: extent of lung injury = (52.7–0.1) × distance 6MWT. A multivariate model was also obtained for predicting the extent of lung injury in SSc, in which the patient’s immunotype, distance in the 6-minute walk test, saturation after the 6-minute walk test, and the presence of dyspnea became the most effective.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> – определение факторов, которые способствуют прогнозированию объема поражения легких (ОпЛ) у пациентов с системной склеродермией.</p><p><bold>Материалы и методы</bold>. В анализ включались пациенты с системной склеродермией (ССД), наблюдаемые в рамках Регистра миозитов, системной склеродермии и смешанного заболевания соединительной ткани (РЕМИССиС), которым была выполнена компьютерная томография высокого разрешения (ВРКТ) легких. Для иммунологической характеристики всем пациентам проведено тестирование на антитопоизомеразные (анти-Scl-70) и антицентромерные (анти-CENP-B) антитела и антитела к топоизомеразе (анти-Pm-Scl).</p><p><bold>Результаты. </bold>В исследование вошли 79 пациентов с ССД (94,9 % женщин), средний возраст – 64,4 ± 11,5 года. признаки интерстициального заболевания легких (ИЗЛ), по данным ВРКТ, отмечены у 50 пациентов. Наибольший объем поражения легочного интерстиция выявлен у пациентов с ССД без склеродермы (32,7 ± 29,3 %), меньший – у пациентов с диффузной ССД (16,9 ± 17,1 %) и самый низкий – у пациентов с лимитированной ССД (8,5 ± 14,2 %). помимо типа заболевания объем поражения ткани легких у пациентов с ИЗЛ, ассоциированным с ССД, был статистически значимо выше при артралгиях, одышке, наличии антител к топоизомеразе I типа и сочетании различных аутоантител. Также установлена статистически значимая обратная связь со всеми показателями теста с 6-мин ходьбой (ТШХ) и форсированной жизненной емкостью легких и прямая связь – с показателями систолического давления в легочной артерии. при оценке связи ОпЛ со степенью одышки по боргу выявлено, что у больных, оценивших одышку менее чем в 3 балла, ОпЛ составил менее 25 %. благодаря высокой степени корреляции создана регрессионная формула зависимости ОпЛ от дистанции в ТШХ: ОпЛ = (2,7–0,1) × дистанция ТШХ.</p><p><bold>Заключение.</bold> В работе получена многофакторная модель прогнозирования ОпЛ при ССД, наибольшую эффективность в которой приобрели показатели иммунотипа пациента, дистанции в ТШХ, сатурации после ТШХ и наличие одышки.</p></trans-abstract><kwd-group xml:lang="en"><kwd>systemic sclerosis</kwd><kwd>interstitial lung disease</kwd><kwd>extent of lung injury</kwd><kwd>6-minute walk test</kwd><kwd>autoantibodies</kwd><kwd>cross-sectional observational study</kwd><kwd>multivariate model</kwd><kwd>high-resolution computed tomography of the lungs</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>системная склеродермия, интерстициальное заболевание легких, объем поражения легких, тест с 6-мин ходьбой, аутоантитела, одномоментное наблюдательное исследование, многофакторная модель, компьютерная томография легких высокого разрешения</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Bergamasco A., Hartmann N., Wallace L., Verpillat P.
