Patients were divided into three age groups: 65–74 years old, 75–84 years old, 85 years old and older. Diagnosis of sarcopenia was consistent with the recommendations of EWGSOP (2010). Muscle mass was measured using dual-energy absorptiometry (DXA) based on the calculation of the AMM index (kg / m2 ). Muscle strength was measured using а grip strength, and physical performance was measured according to the results of a short set of tests of physical activity SPPB tests (short physical performance battery). Cases of falls and fractures were analyzed using a survey and medical documentation.</p><p><bold>Results.</bold> 45.2 % of patients had falls within 12 months before the start of the study, in patients in the age group of 85 years and older the risk of falls is 1.50 higher (95 % confidence limit (CL) 1.24–1.83, p &lt;0.001), compared with the age group younger than 75 years. Falling and non-falling patients had low values of muscle strength and physical performance (p &lt;0.001). Sarcopenia was detected in 30 % of patients. Sarcopenic patients fell significantly more often than non-sarcopenic patients: 89.9 % (95 % CL: 81.5–95.9) and 26.1 % (95 % CL: 19.6–33.2), respectively (p &lt;0.0001). The frequency of falls increased with age mainly in the group of sarcopenic patients (p &lt;0.0001). 30 % of patients had fractures of any localization, 37.7 % of sarcopenic and 26.7 % of non-sarcopenic patients (p &gt;0.05). There were no significant differences in the frequency of fractures in sarcopenic and non-sarcopenic patients in men and women (p &gt;0.05). Fear of falls increased the risk of sarcopenia 3.02 (95 % CL: 1.79–5.11) times (p &lt;0.0001).</p><p><bold>Conclusion.</bold> Sarcopenia significantly increases the risk of falling. There were no differences in the number of fractures and their localization in sarcopenic and non-sarcopenic. Fear of falls is significantly higher in sarcopenic patients.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> – провести анализ случаев падений и переломов у пациентов старших возрастных групп, выявить связь с саркопенией в зависимости от пола и возраста в изучаемой выборке пациентов.<bold>Материалы и методы.</bold> 230 пациентов 65 лет и старше (средний возраст 74,0 ± 6,5 года), наблюдавшихся амбулаторно, включены в исследование. Пациенты были распределены на 3 возрастные группы: 65–74 года, 75–84 года, 85 лет и старше. Диагностику саркопении проводили в соответствии с рекомендациями EWGSOP (2010). Мышечную массу измеряли с помощью двухэнергетической абсорбциометрии (DХА) на основании расчета индекса аппендикулярной мышечной массы (кг / рост (м2 )). Оценку мышечной силы проводили с помощью кистевого динамометра, мышечной функции – по результатам краткого комплекса тестов физической активности SPPB-тестов (short physical performance battery). Случаи падений и переломов анализировали на основании опроса и медицинской документации.<bold>Результаты.</bold> 45,2 % пациентов имели случаи падений в течение 12 мес до начала исследования. У пациентов в возрастной группе 85 лет и старше риск падений в 1,50 раза выше (95 % доверительный интервал (ДИ) 1,24–1,83, р &lt;0,001) по сравнению с возрастной группой моложе 75 лет. Пациенты со случаями падений и без них имели низкие значения мышечной силы и мышечной функции (р &lt;0,001). Саркопения выявлена в 30 % случаев в изучаемой выборке. Пациенты с саркопенией достоверно чаще падали, чем больные без саркопении: в 89,9 % (95 % ДИ 81,5–95,9) и 26,1 % (95 % ДИ 19,6–33,2) случаев соответственно (р &lt;0,0001). Частота случаев падений увеличивалась с возрастом, преимущественно в группе пациентов с саркопенией (р &lt;0,0001). У 30 % пациентов в изучаемой выборке зафиксированы переломы различной локализации, из них у 37,7 % больных с саркопенией и 26,7 % пациентов без саркопении (р &gt;0,05). Не было выявлено различий в частоте переломов разной локализации у больных с саркопенией и без саркопении как у мужчин, так и у женщин (р &gt;0,05). Риск саркопении в 3,02 (95 % ДИ 1,79–5,11) раза выше у пациентов, испытывающих страх падений (р &lt;0,0001).<bold>Заключение.</bold> Наличие саркопении значимо повышает риск падений. Не было выявлено различий в количестве переломов и их локализации как у пациентов с диагностированной саркопенией, так и у тех, у кого саркопения не была выявлена.
