Objective: study was analysis of long-term results of medical and surgical treatment of patients with severe chronic heart failure (CHF).

Materials and methods. We studied 90 patients with CHF III–IV functional class (FC) of NYHA, treated in A.N. Bakulev Scientific Center for Cardiovascular Surgery in 2007. All patients were divided into 3 groups: 30 patients who underwent conventional medical therapy (MT); 30 patients who underwent cardiac resynchronization therapy (CRT); 30 patients extremely severe category, which was performed orthotopic heart transplantation. Patients were followed up for 5 years to assess long-term results of treatment of CHF.

Results. In patients with severe CHF, CRT (with respect to MT) significantly reduces the 5-year risk of total mortality, death from progressive heart failure, rehospitalization for heart failure, cardiac arrhythmias, need for heart transplantation, as well as helping to reduce FC of CHF, and an increase in ejection fraction the left ventricle.

Conclusion. Surgical treatment of patients with severe CHF demonstrated a significant advantage over conventional MT in terms of improving the 5-year forecast.

Цель исследования – анализ отдаленных результатов медикаментозного и хирургических методов лечения пациентов с тяжелой хронической сердечной недостаточностью (ХСН).

Материалы и методы. Исследованы данные 90 пациентов с ХСН III–IV функционального класса (ФК) по классификации Нью-Йоркской ассоциации кардиологов (NYHA), проходивших лечение в НЦССХ им. А.Н. Бакулева в 2007 г. Все пациенты были разделены на 3 группы (по 30 человек): 1-я – группа стандартной медикаментозной терапии (МТ), 2-я – группа сердечной ресинхронизирующей терапии (СРТ) и 3-я – пациенты крайне тяжелой категории, которым была выполнена ортотопическая пересадка сердца. Пациенты находились под наблюдением в течение 5 лет для оценки отдаленных результатов лечения ХСН.

Результаты. Группа пациентов с тяжелой ХСН, находящихся на СРТ, по сравнению с группой больных, получавших МТ, характеризовалась в течение 5-летнего наблюдения более низкой частотой общей летальности (7 % против (vs) 37 %), смертности от прогрессирования ХСН (0 % vs 37 %), повторных госпитализаций по поводу ХСН (10 % vs 70 %), нефатальных сердечно-со- судистых событий (0 % vs 13 %), необходимости в трансплантации сердца (10 % vs 63 %), а также более высокой долей паци- ентов, у которых на фоне лечения наблюдали снижение ФК ХСН (87 % vs 33 %). Пациенты с пересадкой сердца значимо отличались от группы, находящейся на МТ, меньшей частотой повторных госпитализаций по поводу ХСН (37 % vs 70 %), трансплантаций сердца (в данном случае повторных – 23 % vs 63 %) и большей частотой улучшений ФК ХСН (77 % vs 33 %).

Заключение. Хирургические методы лечения пациентов с тяжелой ХСН демонстрируют существенное преимущество перед общепринятой МТ с точки зрения улучшения 5-летнего прогноза.
