Patient A., born in 1988, was followed up and got a treatment more than 2 years in oncology out-patient clinic diagnosed with “Melanoma of the front surface of the left leg T2bN0M0 IIA”. Subsequently, the patient was verified metastasis in the inginal lymph nodes, in the soft tissues of the hips, to liver. Acute adverse reaction has developed in a short time after getting the anti-tumor target therapy as generalized acneform rush and itching of the skin. Skin symptoms accompanied by pronounced psychological and emotional stress, therefore, dermatologists have been invited to provide additional medical assistance to this patient. Due to the fact that subsequent traditional anti-acne algorithms of topical and oral treatment was not such effective, there was made a decision to use an alternative supporting external therapy, which did not have similar examples of usage previously. </italic> <bold><italic>Results. </italic></bold><italic>External application of tacrolimus ointment in combination with other drugs and then as a mono-therapy, allows us in a rather short period achieve a stable and pronounced regression of skin pathological lesions, to return to the previously cancelled initial drug dose of the anti-tumor target therapy, to change significantly components of the patient’s quality of life. </italic> <bold><italic>Conclusion. </italic></bold><italic>The search for additional and alternative treatment approaches for similar patients, as in our case, remains relevant for specialists and patients themselves. This case is an example of alternative approach to the tacrolimus topical application in patient with drug-mediated acneform rush.</italic></p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Цель работы </italic></bold><italic>– описать результаты совместного наблюдения и диверсифицированного лечения онкологами и дерматологами пациентки с распространенной рецидивирующей меланомой кожи, которая получала в течение длительного периода таргетную противоопухолевую терапию, сопровождающуюся выраженным побочным эффектом в виде распространенных и резистентных к традиционному лечению акнеформных высыпаний. </italic> <italic>Пациентка А., 1988 г.р., более 2 лет наблюдалась и проходила лечение в медицинской организации онкологического профиля с диагнозом: «Меланома передней поверхности левой голени T2bN0M0, IIA стадия», а в последующем и с верифицированными метастазами в паховые лимфатические узлы, в мягкие ткани бедра, в печень. На фоне терапии противоопухолевым таргетным препаратом – ингибитором митогенактивируемых регулируемых внеклеточными сигналами киназ – у пациентки в короткие сроки развилась острая побочная реакция в виде генерализованных папуло-пустулезных высыпаний, зуда кожного покрова. Дерматологическая симптоматика сопровождалась выраженным психоэмоциональным стрессом, поэтому к оказанию медицинской помощи в виде назначения сопроводительной терапии были привлечены врачи-дерматологи. В связи с тем, что назначения по традиционным схемам топической и пероральной терапии недостаточно эффективно купировали проявления побочного эффекта таргетного препарата, было принято решение о применении альтернативного способа сопроводительной наружной терапии, ранее не имевшего аналогичных примеров использования. </italic> <bold><italic>Результаты. </italic></bold><italic>Наружное применение препарата такролимус в схемах комбинированной, а затем и монотерапии позволило в довольно короткие сроки добиться выраженного и стабильного регресса патологических высыпаний, а также возврата к ранее отмененным первоначально получаемым дозам онкологического таргетного препарата, значимого изменения составляющих качества жизни пациентки. </italic> <bold><italic>Заключение. </italic></bold><italic>На современном этапе развития медицины поиск дополнительных и альтернативных подходов к лечению онкологических больных по-прежнему остается актуальным как для профильных специалистов, так и для самих пациентов.
